You will need the following information to submit a claim

• Copy of marriage certificate               
• Applicable for Motshidisi claim
• Birth certificate for minor                     
• Proof of bank details                                
• Payslip or Bank Statement

Email subject: Funeral Benefit Claim (with applicant ID number)

2. How to claim for your Cashback benefit

You will need the following information to submit a claim
